THE FOLLOWING INSTRUCTIONS ARE FOR UO FOUNDATION SYSTEM USERS
To request access to UO Foundation systems or data, please follow the steps outlined below.
The Process For Requesting Systems Access
- The individual requesting access fills out the Systems Access Request Form and Technology Use & Confidentiality Agreement.
- The requestor obtains supervisor signature.
- After supervisor approval, the request is routed to an access grantor via the Helpdesk.
- Once access is granted, the requestor and/or supervisor will be notified via email.
Please Note That Your Request Cannot Be Fulfilled Unless:
- All applicant information on Page 1 of the Systems Access Request Form is complete. Be sure to include and verify the required fields of Name, Phone Number, UO ID Number, UO E-mail Address, Job Title, User Role/Job Function, and Campus Affiliation are accurate.
- All system access needs information on Page 1 of the Systems Access Request Form is complete.
- A valid Department/Unit Authorized Signer has signed Page 2 of the Systems Access Request Form. Note: If requesting access after a change in role, department, and/or unit, this must be a "wet" signature as permissions may have also changed. Docusigned forms will not be accepted in these cases.
- The employee has reviewed and signed the attached UOF Technology Use & Confidentiality Agreement.
Common Reasons For Requesting Systems Access
- New employment
- Change in employment (role, department, unit)
- Account disabled due to inactivity
Enabling and Terminating Access
The UO Foundation Technology Services department will enable access based on the signed Systems Access Request Form and Technology Use & Confidentiality Agreement for the UO Foundation systems, including Litmos, Okta, Advance, ABI, Laserfiche and Imodules.
Access to these systems will be terminated based on:
- Separation from the UO Foundation
- Change in job duties recorded in the HRIS system
- Instructions from Department/Unit heads.
